 A. Making the Decision to Transfer to Unifour Pediatrics:
Small Ball We thank  you for transferring to Unifour Pediatrics.
    Here are some documents which should help you with your decision:

Small Ball Read our Immunization Policy and Behavior Policy before you transfer.
B. Obtain Transfer Records from Other Practices:

 Small Ball Request to Transfer from other practice - Download/Print Out Now.
Small Ball Fill out and sign these transfer forms (one for each child to transfer).
Small Ball Send these transfer forms to the other office or to our office.
Small Ball Try to complete this process well before the first visit, because it
     takes time to receive transferred records, and without the past medical
     records, this significantly prolongs the visit and decreases the quality.


 C. Read, Fill Out, and Sign ALL Patient Intake Forms:

Small Ball Download All 8 New Patient Intake Forms - Download/Print Out Now.
      (we have included all 8 forms needed, to simplify your transfer here. By
       filling our forms prior to the visit, minimal forms to fill out at your first visit)
Small Ball Read, complete, and sign all forms - this will save time later.
 Small Ball Then, deliver these to our office:
     either mail these to us -OR- FAX them to us -OR- hand deliver them to
     our office (see Below).
Small Ball Try to complete this process well before the first visit, because this
     will save you from having to fill out all of these forms in the waiting room
     before being seen.

 D. Submit these forms to us (Any of these methods):

 Small Ball Hand deliver to: 3411 Graystone Place, Conover, NC 28613.
